A study published in Scientific Reports has found that accelerated biological aging in midlife is associated with an increased risk for dementia in women, but not in men. The research drew on data from the Framingham Offspring Study, tracking more than 3,000 participants for a median of 26 years.
Biological age refers to the functional state of a person’s cells and systems, which can differ from chronological age, according to the study authors. Participants whose biological age exceeded their chronological age were categorized as having accelerated biological aging. The analysis examined links to cardiovascular disease, dementia, and all-cause mortality over the long term.
Researchers estimated biological age from clinical biomarkers including total blood cholesterol, blood sugar, and scores on the Mini-Mental State Exam, a standard cognitive screening test. Participants were placed into one of three categories: accelerated, decelerated, or concordant biological age relative to their chronological age.
Among those with accelerated biological age, men were on average 4 years older biologically than their chronological age, while women were 2.5 years older. The majority of participants fell into the concordant category, meaning their biological age roughly matched their calendar age. Covariates included diabetes, blood pressure medication use, and smoking status. The study was observational and cannot prove causation, the authors noted.
During follow-up, there were 713 cardiovascular disease events, 1,105 deaths, and 265 cases of dementia. Participants with accelerated biological age faced a 1.6-fold higher risk for cardiovascular disease in men and a 1.8-fold higher risk in women, compared with those who had decelerated biological age. Both men and women with accelerated aging had a 2-fold greater mortality risk.
For women, accelerated biological age was linked to a statistically significant increase in dementia risk. Sensitivity analyses confirmed that women with accelerated or even concordant biological age were at higher risk relative to those with decelerated aging. For men, no significant dementia risk was found. Nearly two-thirds of Alzheimer’s disease patients are women, a disparity that researchers have long sought to explain, according to a separate study in Alzheimer’s Research & Therapy [1].
The study’s observational design means it cannot establish cause and effect, and biological age remains a proxy measure with no standardized calculation method. Dr. Salim Hayek, a professor of cardiovascular medicine at the University of Texas Medical Branch who was not involved in the research, noted that adding biological age to existing risk models improved prediction only marginally. “The change in the c-statistic ran from essentially zero to about 0.01. So the measure tracks risk, but it isn’t telling you much beyond what the individual risk factors already say,” Hayek said.
Dr. Cheng-Han Chen, a board-certified interventional cardiologist at MemorialCare Saddleback Medical Center, stressed the need for standardized clinical definitions before routine use. “We are not yet at the point where biological age should be used as a routine clinical tool,” Chen said. Beverly Pennington, a geriatrics nurse practitioner at UTMB, said she would want to know what factors caused accelerated biological aging, such as education, substance abuse, and environmental factors.
Researchers suggest that biological age could help identify high-risk individuals not captured by a single biomarker, but it is not ready for clinical application. Experts emphasized that treating underlying risk factors — blood pressure, smoking, diet — remains the primary approach. “There is no therapy for accelerated aging; you treat the components, and the components are the same ones we’ve been treating for 40 years,” Hayek said.
Large-scale studies have found that avoiding high blood pressure, diabetes, and smoking in midlife could add 13 dementia-free years [2]. Other research links dietary fiber intake to lower dementia risk [3] and moderate exercise to better brain health decades later [4]. Future studies should examine why the sex difference in dementia risk appears and what drives accelerated biological aging. Pennington noted that modifiable lifestyle factors such as sleep quality, stress, and environmental chemical exposure may play a role [5][6].
